I am a provider for Blue Cross Blue Shield PPO, Anthem Blue Cross PPO, and Medicare Part B.

I am out of network for all other insurance plans.

I am not a Medicaid, Blue Cross Community, or Care Source provider.

I do not accept Medicare Advantage plans.

If you want to try and use your health insurance please do the following:

1.  Call the member services number on the back of your insurance card.

2.  Ask what your outpatient mental health benefits are in an office setting.

3. If your plan is not a Blue Cross Blue Shield PPO or Anthem PPO plan ask what your “Out of network” benefits are for out patient mental health in an office setting.”

4. Ask what your deductible is for the year for outpatient mental health in an office setting and how much of it has been met. Ask if your deductible starts over in January or at a different time in the year.

5. Ask if you have a co-pay or co-insurance payment for sessions and what that amount is.

6. Ask if services need to be pre-certified or pre-authorized. If yes, have them do that for you. Ask how many sessions are approved and for the authorization number.

7. Ask how many sessions are allowed per calendar year.
